The Scheduling Department will schedule the following
services:
• Ultrasound
• Nuclear Medicine
• Vascular Ultrasound
• CT Scan
• Invasive Special Procedures
• Fluoroscopy |
When calling to schedule an appointment please
have the following information available:
• Spelling of first and last name
• Date of birth
• Social security number
• Tests that are required
• Name of Physician ordering tests
• Diagnosis-reason for test
• Insurance information
• Insurance authorizations |
At time of scheduling, your office will be notified
if there is a special prep for the procedure.
Registration: Advise the patient
to register at Physician’s Hospital one hour
before appointment. They enter Physician’s
Hospital through the main entrance and proceed to
the inpatient/outpatient registration area.
What to expect: A Patient Accounts
Representative will register the patient and will
verify information. A copy of their identification
card and insurance card will be obtained. The patient
will then be taken to the appropriate testing area. |
